Do you need to update your program?

If you are a graduate studentÌýwho meets one of the following criteria,Ìýyou mustÌýupdate your program.

• 
  Students must use this form if they meet one of the following criteria:

  • Student wishes to change a program choice on a currently active application
  • Student wishes to add to a current program (i.e. a license, endorsement, certificate, or additional graduate degree)

Please note: Programs may require additional application materials. Visit Graduate Programs to check your application status and submit additional materials.

  Students must use this form if they meet one of the following criteria:

• • Student wishes to return to theirÌýprevious program and graduate status only after having discontinued course work for a period longer than two years

Please note:ÌýStudents who wish toÌýbegin an entirely different programÌýor status mustÌýÌýContact theÌýSchool of Graduate StudiesÌýif you have issues reapplying.

• Reapplication
  Students must use this form if they meet one of the following criteria:

  • Student wishes to complete an application after canceling/discontinuing an application or admission status overÌýa period longer than one year
  • Student wishes to pursue a graduate program after withdrawing from either the same or different program
  • Student wishesÌýto pursue a different program after completing a previous program

  Students must use this form if they meet one of the following criteria:

  • Student wishes to cancel a current application
